MEDIA CONSENT & RELEASE

I, the undersigned, hereby grant Fix It With Piggy and its representatives permission to photograph, film, and/or record my child named below, and to use the resulting images, video, and/or audio for educational, promotional, and informational purposes (including print, web, social media, and presentation materials), without compensation to me.

I understand that my child's first name may be used; last names will not be used publicly without explicit additional consent. I release Fix It With Piggy from any liability related to the use of these recordings.
